Families must use this form to enter the CSF lottery. Scholarship recipients are chosen through a process of random selection. Entering the lottery does not guarantee that a family will be selected to receive scholarship assistance. Click here to see if you qualify.

Scholarship Estimation Worksheet
The scholarship estimation worksheet should not be submitted to the office. After a family has been awarded a scholarship, this form helps determine what the amount of the CSF scholarship will be and what remaining tuition amount they will be responsible for paying to the school.

School Commitment Form
Families must take the School Commitment Form to the school(s) each scholarship recipient will attend and the form(s) must be completed by the school principal(s). A School Commitment Form must be completed for each child who will receive scholarship assistance. This form lets CSF know what school a scholarship student will attend, what the tuition costs will be and when the student will begin enrollment at the school.

Homeschool Commitment Form
This form is used by families who will homeschool their children and must be filled out completely, including the information regarding the curriculum that will be used. Families must also submit a copy of their certificate from the NC Division of Non-Public Education, verifying homeschool registration.

Requalification Form
The Requalification form is to be filled out by the parent(s) each year. This form serves as a notice to continue in the program. If you will not be using a scholarship, please let us know by answering all questions.

Sibling Form
The sibling form is to be filled out by families who have children entering grades K-8 but are not currently receiving a CSF scholarship. As the siblings of current recipients, these children are also eligible for a scholarship if they are:
- legal dependents of the parent,
- at least 5-years-old and
- entering
grades
Kindergarten
(not
pre-K) – 8
in
the
Fall.

Exit Confirmation Form
This form is completed by the school principal or other school official when a CSF student leaves the school, informs the school of plans to leave, or fails to re-register at the school.
